I installed the plugin as indicated and restarted. The plugin is marked as active, but Ctrl+H does not work and Search & Replace is not present.

What am I doing wrong? What can I do?

Can you open it through the menu (Edit -> Search and replace)? Perhaps the hotkey fails to register.

Have you installed any other plugins?

(Also sometimes, opening the settings then closing it, fixes issues.)
