Produce a report that summarizes Pass/Fail for each QC check for each sample

We'd like to be able to determine at a glance if any samples failed QC checkpoints in the pipeline.

Produce a summary file that reports Pass/Fail status for each sample, for each QC checkpoint. The summary file can then be loaded into some sort of grid view that will be easy to identify QC failures for a full multi-sample run.
