Thanks for reaching out. I believe @gabesequeira is working on this issue but I forgot to assign it to him. Sorry! In terms of fields, we should start with most basic functionality needed. We do not have a front end design yet so lets start with just creating the form. @krittiyaclark I will follow up with you on slack.
