Almost three years later you may not remember, but. Was that a BtrFS volume with compression enabled?

I've recently realized that there are no separate lengths for physical and logical lengths in FIEMAP API, so for compressed volumes fragview will show extremely wrong mappings. Including files apparently spanning beyond volume borders.

Nope, that's Ext3. I never had BTRFS anywhere.

Oh, okay. Then that's a bug in fragview.
