"could not autofill fields" error is showing up #2

when trying to update logupdate using voice autofill feature, often its throwing an error message. "could not autofill fields"

steps to reproduce

open a patient card and click on logupdate
select round type
click on Voice autofill button
record the patient health updates
click on stop recording
see the error

This error is occuring randomly @shivankacker can you look into it ASAP
