I don't use Ivy but I don't see why it wouldn't be possible. The search interface is configurable through org-super-links-search-function and there are a few notes about how it could be added here https://github.com/toshism/org-super-links#org-super-links-search-function

If anyone wants to give it a try i'm happy to help advise.

I think it would probably be better to rely on the built-in org mode function for finding refile targets by default. That should have generally the same functionality everyone wants, as long as completion is implemented for each of the link styles that you want to use, which you should be doing anyway for a number of other reasons for best integration with org mode, and the built-in org functions should use whatever completion framework you have installed by default anyway, shouldn't they?
