Official Release - 2.0

This is finally an official release with help from @simsterm. It is compatible with Flow 5.0 and upwards including dev-master and can be used with monolog versions 1.23 and 2.3, so should be pretty compatible to everything.

Also included a bit more documentation in the readme.
